Care Plans

With Care Plans, your GP has recommended you speak with a dietitian to make dietary changes to improve or better manage your health.

When making an appointment to see the dietitian please note:​

  • Initial appointments are 45 minutes in length (please bring your care plan and referral letter.)

  • Initial sessions are $90.35, you will be rebated $60.35 by Medicare.

  • Follow up appointments on a care plan are bulk billed and 30 minutes, additional visits not covered by the care plan are $60.​​

Private Referrals

Clients can also privately self-refer. You may have questions on how to make dietary changes to assist with sporting performance or learn about healthy eating patterns. â€‹

 

​When booking an appointment to see the dietitian please note:

  • You will be required to pay $95 for the initial 45 minute consult. (Check if your private health insurance with extra’s cover allows you to claim a portion of the fee).

  • Follow up consults are $60 and 30 minutes.

Please keep a food diary for 3 days or make a note of common foods you eat throughout the day before attending the initial appointment.

A written food diary or mobile phone apps such as Easy Diet Diary or See How You Eat can be a useful way to record what you have been eating. 

A Note on Meal Plans: During your visit the dietitian will complete an assessment to see what you eat and will provide recommendations based on your current eating patterns. Your dietitian will give you the knowledge and tools to make healthy food choices to achieve your nutrition goals. Some clients may require additional support and we can work with you to create a meal template/guide in a separate appointment, but dietitians do not hand out meal plans. The goal is to teach clients to use their nutrition knowledge to make decisions on what to eat, each and every day.
